📢 મહત્વપૂર્ણ: UDID Card માટે disability type અથવા percentage applicant પોતે નક્કી કરતો નથી. Assessment અને certification authorized Medical Authority / Medical Board દ્વારા કરવામાં આવે છે.

♿ UDID Card શું છે?

UDID એટલે Unique Disability ID. Department of Empowerment of Persons with Disabilities, Ministry of Social Justice & Empowerment દ્વારા Persons with Disabilities માટે national database અને unique identity system તરીકે UDID project અમલમાં છે.

Portal મારફતે Disability Certificate અને UDID Card માટે online application, application tracking, renewal/update, replacement તથા certificate download/print જેવી services મળી શકે છે.

✅ કોણ અરજી કરી શકે?

  • Specified disability ધરાવતા વ્યક્તિ.
  • Medical assessment માટે જરૂરી information/document આપી શકે તેવા applicant.
  • Adult અને child applicant માટે applicable identity/Aadhaar requirements પૂર્ણ કરવી.
  • Applicationમાં residence/districtની સાચી વિગતો આપવી.
Official DEPwD information મુજબ system હાલમાં 21 specified disabilities support કરે છે, પરંતુ applicable law/guidelinesમાં future update થઈ શકે છે.

📄 અરજી પહેલાં શું તૈયાર રાખવું?

Portalમાં final document requirement applicant અને case પ્રમાણે દેખાય છે. સામાન્ય તૈયારી તરીકે:

  • Aadhaar / applicable identity details
  • ચાલુ Mobile Number
  • Applicantનું નામ, Date of Birth અને Address
  • Recent photograph
  • Existing Disability Certificate હોય તો તેની details
  • Medical records / reports જ્યાં applicable હોય
  • Address / residence સંબંધિત supporting information

💻 UDID Card Online Apply Process

Official UDID Portal ખોલોswavlambancard.gov.in ખોલો અને Apply For UDID option પસંદ કરો.
Applicant Registration કરોમાગેલી identity, mobile અને personal details દાખલ કરો.
Application Details भरोAddress, disability-related information અને district details સાચી भरो.
Documents Upload કરોPortalમાં જે documents mandatory બતાવે તે readable formatમાં upload કરો.
Medical Authority પસંદ/identify કરોPortal process મુજબ concerned Medical Authority / CMO office સાથે application જોડાય છે.
Application Submit કરોFinal submit પછી Enrollment/Application Number સાચવી રાખો.
Medical Assessment Attend કરોScheduled date પર required documents સાથે હાજર રહો.
Medical Board ReviewAuthorized doctors disability type અને percentage assess કરે છે.
Certificate / UDID GenerationApproval પછી Disability Certificate અને UDID generate થાય છે.

🏥 Medical Assessmentમાં શું થાય?

Medical Authority applicantની conditionનું assessment કરે છે. Specialist doctorનો report અને Medical Board reviewના આધારે applicable disability type અને percentage determine થાય છે.

માત્ર online form submit કરવાથી UDID Card automatically approve થતું નથી. Medical assessment અને authority decision જરૂરી હોઈ શકે છે.

Official Track Application pageમાં નીચેની વિગતોમાંથી applicable detailથી status search થઈ શકે છે:

  • UDID Number

🔄 Renewal / Update / Lost Card

UDID Portalમાં Disability Certificate / Card renewal, information update અને Lost UDID Card સંબંધિત self-service options ઉપલબ્ધ છે. Existing PwD login માટે Enrollment Number અથવા UDID Number અને Date of Birth જરૂરી થઈ શકે છે.

❌ સામાન્ય ભૂલો

  • Name અને Aadhaarમાં અલગ spelling રાખવી.
  • Wrong Date of Birth ભરવી.
  • Incorrect district/address પસંદ કરવો.
